If you are interested in diving into your wellness journey with us, please take a moment to fill out this new patient form and send it to capitolwellnesscenter@gmail.com. *While Dr. Breen is currently not scheduling new patients, we highly recommend sending in this information so that when we do start scheduling, the process will be easy to get you in. If you plan on using insurance, please send a copy of your insurance cards along with your new patient form.

Please fill out a new patient form for any service or program provided by Dr. Been and list what service in the bottom section.

Natural Glow and Kate's patients do no need to fill out this form. Please just call the office to schedule with her.
